Forms and form fields exist throughout your Zendesk Help Center.

Form selection

Available on the New Request page and New Community Post pages, this pattern presents your forms or topics in a unique and interesting way. By default forms are presented in a list, styled as a dropdown, but they can be presented as blocks or list items.

Multiple forms are only available to Zendesk customers with Professional add-on and Enterprise accounts.

Use snippet-replace-form-options.js with one of the following templates on the new_request_page.hbs template to instantly improve the way that your vistors interact with your forms.

Search field

Search fields can be revealed on click instead of being visible at all times. The following two examples demonstrate search fields that are revealed when a search icon in the header is clicked.
